Services

What are Individual Supports Services?

Services that provide direct support and assistance for participants, with or without the caregiver present, in or out of the participant’s residence, to achieve/or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community. Individual Supports includes:

  • Assistance with community based activities
  • Training/supervision
  • Basic self-care
  • Social skills
  • Activities of daily living

Eligibility

Eligibility for DDD services

To be determined eligible for DDD services, an individual must:
 Be a New Jersey resident
 Be Medicaid eligible
 Meet the functional criteria of having a developmental disability, and must document that s/he has a chronic physical and/or intellectual impairment that
ᴑ Manifested in the developmental years, before age 22;
ᴑ Is lifelong; and
ᴑ Substantially limits the individual in at least three of the following life activities: self-care; learning; mobility; communication; self-direction; economic self-sufficiency; and the ability to live independently

Medicaid Eligibility for DDD Services

To be “Medicaid eligible” means that an individual has applied for and been enrolled in Medicaid and continues to meet the income and financial resources criteria for Medicaid. Every New Jersey resident who qualifies for and receives federal SSI (Supplemental Security Income) automatically receives New Jersey Medicaid. Therefore, it is highly recommended and strongly encouraged that individuals with intellectual and developmental disabilities immediately apply for SSI when they turn 18.

Independent Services

Self-Directed services are provided by Community Mainstreaming to assist people with IDD in maintaining their independence and making decisions about their future. Our skilled experts offer support to reach their full potential.

What Is Self-Direction?

Under the HCBS waiver, self-direction is an option that gives people with developmental disabilities and their families the freedom to select the supports and services that best fit their specific objectives. Self-Direction empowers people to make choices about their development, relationships, and activities, from getting a job to volunteering, living independently to upholding a healthy lifestyle. Circle of Assistance

Bella Rose offers a special program called Self-Direction, which gives people the tools they need to take charge of their own life with the help of their Circle of Support. Family members, advocates, brokers, care coordinators, and the individual themself usually make up this circle. The Circle of Support’s main responsibility is to assist the person in carrying out their selected life plan. Bella Rose is pleased to provide both Fiscal Intermediary and Support Broker services, should self-direction be of interest to you.

A Fiscal Intermediary: What Is It?

Bella Rose serves as a Fiscal Intermediary (FI) for people who have made the decision to manage their own finances. Working closely with the individual, family, and support broker, as a Fiscal Intermediary, we will execute the “behind the scenes” aspect of all HCBS waiver services. In addition to billing and paying for services that the individual has selected and identified, services frequently involve human resources tasks like staff training, payroll processing, budget completion and monitoring, Medicaid billing review and audit, and training for the individual, family, and staff regarding billing and documentation responsibilities.

A Support Broker: What Is It?

Employees of Bella Rose Residential can function as agency brokers internally, or we can work with an independent broker of your choosing. In order to help the person and their family through the process and continuously make sure the person receives our incredibly high standard of care, we collaborate closely with agency and contracted Brokers. The Person-Centered Planning (PCP) process is led by the support broker, who takes the person’s requirements, desires, and aspirations into account. Together with the Circle of Support, they assist in developing a habilitation plan and budget, and they offer extra training and assistance as required.
